Sir/Madam,
All candidates who have qualifiedthe Recruitment Test held on 27.04.2014 for the subject post are requested to furnish self attestedphotocopies of documents/certificates in support of their claim for the post alongwith Online Recruitment Application(ORA).

Caste certificate from a Competent Authority in prescribed format as per advertisement indicating clearly your (i) Caste (ii) the Act under which the Caste is recognized as SC/ST/OBC and (iii) the village/town you are ordinarily a resident of. The Issuing Authority of the Caste certificate should be as per the Advertisement. [for SC/ST/OBC candidates]

Degree along with Mark Statements/Marksheets pertaining to all the academic years/semesters in support of your Educational Qualifications as per the Advertisement No. 13/2013 (Item No.
04, Vacancy No. 13091304114). (In case Degree is yet to be conferred upon you, then
provisional Degree/certificate along with marks statements pertaining to all the academic years should be furnished as proof of educational qualifications.

Certificate of Enrolment on State Register or Central Register of Homoeopathy

Certificate of Government Servant (for candidate claiming age relaxation as Govt. Servant) in the prescribed format as per the Advertisement

Certificate of Physically Handicapped Persons in the prescribed format as per the Advertisement

An Affidavit if there is difference in either name/father’s name or spelling of name/father’s name in all the Certificates with reference to Online Recruitment Application (ORA).

Important: Originals of the above certificates are required to be produced at the time of interview.
